Patient Information

New Patients
On registering, all new patients are invited to see the Practice Nurse for a “check up”. The Practice has a new Patient Policy. A copy of this will be given to you when you ask at register.
Sick Notes
For absences of 7 days or less you do not need a certificate from the Doctor. Ask the receptionist for a self certification form. If you require evidence of sickness for less than 7 days you may ask the doctor for a private certificate, for which a fee will be charged.
Non NHS Examinations
Medical examinations for special purposes (employment, driving, etc.) are done outside surgery times. A fee will be charged.
Medical Students
Commonfield Road Surgery is a recognised teaching centre for the University of Liverpool. On a regular basis medical students, as part of their training, may be present with your Doctor during consultation. If you object to this please tell the receptionist or Doctor.
Zero Tolerance
The Doctors, staff, patients and visitors in this Practice have the right to work and be cared for in a safe and supportive environment. Both verbal and physical violence against our staff is a crime and we will not tolerate anyone who behaves in a violent or abusive way. Anyone behaving in such a manner will be off-listed. The Practice will contact the Police if we feel that it is appropriate.
The Practice Area
The practice serves Woodchurch and the surrounding districts of Upton, Greasby, Prenton, Arrowe Park and Thingwall. A map is available on request.
If you change address please inform us – if you are outside the practice area you may be asked to change to another surgery.

Training
The surgery is closed approximately one afternoon per month for training as protected learning organised by the Primary Care Trust. Notices are displayed in advance to advise of dates and emergency cover is always available by GP out of hours.
